321a1c9dc0 Cleaned up namespace handling. 
A Namespace object is now guaranteed unique.
This cleaned up some old workarounds and latent bugs.

f064a6cc60 Test case for CreateVectorOfStructs() using a callback. 
Fixes issue #4392, also fixed C++98 compatible code generation in tests.
